He will assist you in any way he can. By filling out this Application does not make you a Metis, Non-status, Status, and Inuit groups living off reserves in Ontario. It merely joins you to a group of Indigenous People who want to reclaim our voice, traditions, heritage, spirituality, etc. and walk a path of Indigenous Pride into the Future. OCIP applauds you for continuing on your Indigenous Path with us.

It is of great importance that you ensure that the documentation is clear and well photo copied (Always keep the original Documentation).
Please Note:- Although the New Applications have “Indigenous” in Place of “Aboriginal” ALL required Information and/or Representations are the same.
The signature and date must be Clearly signed in Black Ink.
The Information supplied herein can be of great assistance to other family members. So if a family member is joining, you can add your membership number to assist them and OCIP Membership at any time.
It is of great importance that the Photo be of the highest possible quality.
Also, ensure that the documentation is clear as possible and well photo copied.
Always keep the original Documentation
If you are paying for another Applicant(s) please make a notation here and on their Application as well.
It is important to record any associated numbers that you may know, so if you doso, please make a note here; like- “See Attached”, or “See Over”, etc. As sometimes Copies are hard to read and numbers can be unintentionally missed.
For the sake of clarity, Please note any name changes within Documentation, as this helps with future Family or Related Applications. Especially in the cases of adoption, remarriage, etc. involving Children.
It is Important to Keep OCIPS records as up to date as possible so please update us of any Contact Information changes.
There is a Change of Contact Details Form, that is easy to use and forward to the Membership Liaison Person.
It is of Greatest Importance that You Enclose as much detailed Information as you can find here. Remember to include any numbers and/or all documentation that demonstrates Indigenous Ancestry.

It must be remembered that this Oath of Aboriginal Status and Affiliation is an important step on the path of Self-determination AND reclaiming our heritage. It must not be entered into without thought. However missing information should not be a reason to begin the process.
